Teams introducing care coordination encounter a range of challenges, some of which may deter the team and undermine the success of the programme. This training is delivered in a 1-2-1 coached format and will enable teams to navigate these and other difficulties and realise the full potential of care coordination for patient care and team motivation.

This blended learning programme offers guided learning activities, one to one tutor contact, and assessment of written work, for the preparation and introduction of care coordination systems

This course requires study at level 3. At this level students need to have English Functional Skills at Level 1. If tutor advises that functional skills training is required, this may involve additional functional skills training at my own cost.

Learners must produce coursework in the Microsoft Word Format

The course fee includes 3 months of tuition. Following this additional bungles of tuition can be purchased if required at £50 per month
